Office Policies
	Our staff is well trained to answer most of your questions. We are proud of our staff and hope that you find them helpful. Questions concerning diagnosis and treatments are usually answered by the Doctor.
	Most insurance plans require that lab tests be performed at certain contracted sites. We will assist you in scheduling.
	Generally, x-rays will be taken in our office unless your insurance requires that they be performed at outside site.
	If you need to refill your prescription, please call your pharmacy and have them contact the office for a refill, or call the office during office hours.
	Most health plans require referral from your Primary Care Physician; please make sure that your referral is current.
	If you have to cancel your appointment, please contact the office at least 24 hours prior to your appointment. A charge may be applied if you fail to contact the office and not show up for the appointment.
	Our physicians may be reached after hours by calling our office and leaving a message.
